How it works

• We accept signed standing orders by a primary care provider. We only use your orders for your eligible Peoples Health patients (those you’ve seen within 12 months and for whom we don’t have a record on file for the service).
• Your Peoples Health representative will reach out to you, or you may contact your representative about signing orders.
• Submission options:
  • Quickly and easily sign standing orders through our automated digital process via Adobe Acrobat Sign. Your Peoples Health representative can arrange this for you.
  • Submit a signed standing order form via secure email or by fax to 504-849-6991.

When completing the form, initial next to each service agreed to be covered under the standing order. Signed orders remain active for 12 months from the date of signing
